Introduction
If you need a copy of your birth certificate, there are three ways to get it. You can apply online, by mail, or in person. Online applications for birth certificates don’t require special documents beyond the names and dates of birth for both parents (if these are known). If you’re not sure about these details, though, it’s better to use one of the other methods.
Online.
You can order a copy of your birth certificate online. You will need to pay the fee and provide a valid credit card. You may also be able to request an expedited order, which means that you will receive your birth certificate sooner than usual.
You can also apply for a certified copy of your birth certificate online by visiting the Florida Department of Health website, clicking on “Birth Certificates” at the top of their homepage, then selecting “Obtain A Copy Or Order An Expedited Copy Of Your Birth Certificate Online.”
By mail.
You can get a copy of your birth certificate by mail. To do so, you must fill out an application and pay the appropriate fees. If you are mailing the application yourself, be sure to include enough postage for it as well.

In-person.
You can get a copy of your birth certificate in person at the Florida Department of Health or from your county health department. You’ll need to bring identification and proof of your identity with you, as well as pay for any fees that apply.

You can get a copy of your birth certificate from the Florida Department of Health in one of three ways
If you are a Florida resident, you can order a copy of your birth certificate through the Florida Department of Health. You will need to fill out an application and provide identification before receiving your copy. There are three ways that you can do this:
- Online: Go to http://www1.doh.state.fl.us/vital_statistics/index2.html and follow the instructions on the page to order your certificate by mail or in person at one of their offices throughout the state (you will have to pay an additional fee).
- By Mail: Fill out this form(http://www1.doh.state/forms/vital_records_application_form_new)and send it along with payment for $15 per copy via check or money order made payable to “Bureau of Vital Statistics”.
